GUS PlusPortals + Required School Forms

All required fall forms, including health forms, day trip permissions, and band for grades 5-8, are now accessible through GUS PlusPortals.

Please complete these forms by August 15.

Returning families, your Plus Portals login information will remain the same. All new GUS families will need to set up a new account to access these forms. To-do so, please follow these instructions (illustrated below):

  1. Visit plusportals.com/gus.

  2. Click “Can’t access your account?” to reset your password.

  3. Enter your user name, which is the email address you used to enroll your student.

  4. Check your email for a system-automated message to “reset” your password. Follow the instructions to reset your password.

  5. Login to Plus Portals using your email as user name and your new password.

  6. Navigate to the “Forms” tab to view and submit all required forms for your child(ren). You can also view the school calendar and access the directory.
